3.	Five (5) properly identified 35mm slides or CD/DVD or sketches, computer images or photographs of proposed design of works for consideration
4.	Identification sheets for each of the five works
5.	Self-addressed U.S. Postal Service for return of materials (must have sufficient postage attached)
